Licensing a private clinic in Uganda: supervising doctor and renewal

What the Uganda Medical and Dental Practitioners Council asks of a new or renewing health unit, who may supervise it, and the 31 December licence deadline.

Opening a clinic in Uganda is a medical decision and a regulatory one. The Uganda Medical and Dental Practitioners Council (UMDPC) licenses private health units supervised by doctors, and its published requirements document, dated September 2025, sets out what it wants from a new unit, from a renewal and from the doctor who supervises it. This post summarises those requirements for an owner planning a new clinic or keeping an existing one in good standing.

Registering a new health unit

The UMDPC's requirements for registration of all new health units are:

  1. A duly filled, signed and stamped checklist by the District Health Office or a UMDPC inspector.
  2. A copy of the valid Annual Practising Licence (APL) of the supervising doctor, who must have three years from the date of full registration with the Council.
  3. A duly filled and signed commitment letter by the supervising doctor.
  4. Registration fees as per the UMDPC fees structure.

The document also lists registration of the business or company name as part of the paperwork, so the Uganda Registration Services Bureau certificate belongs in the file before the inspection is booked.

Renewing the licence

Renewal asks for the same checklist, a copy of the supervising doctor's valid APL (again with three years' experience from full registration), the signed commitment letter and the operating licence renewal fees per the fees structure, plus a clear photocopy of the previous operating licence. The inspection checklist is therefore an annual event, not a one-off, and the premises should be kept to the standard the checklist describes all year rather than tidied for the visit.

Who may supervise

The requirements are specific about the supervising doctor. There must be a supervising doctor responsible for the day-to-day operation of the health unit. The doctor must possess the qualifications to supervise the licensed scope of services for that unit, must have practised under the supervision of a senior practitioner for not less than three years after full registration, must commit a minimum of one calendar year to the supervision of the unit, and one doctor may supervise a maximum of two health units in a given calendar year.

For an owner who is not a doctor this is the hardest requirement to meet and the one to settle first. The supervising doctor's commitment letter is a personal undertaking, and the two-unit cap means a popular doctor may already be committed elsewhere.

Higher levels of facility

The same document describes the compulsory services for a general hospital, which include outpatient and inpatient multispecialty care and clinical laboratory services including a blood bank, and states that the supervising doctor of a general hospital shall be a registered specialist with the Council. A tertiary hospital is licensed for multidisciplinary sub-specialty services and is expected to support training supervision. A clinic owner planning to grow should read these sections early, because the step from clinic to hospital changes the supervising requirement, not just the building.

The doctor's own licence and the 31 December deadline

The clinic licence depends on the supervising doctor's APL, so the doctor's renewal calendar is the clinic's calendar. According to the UMDPC document, the first APL is issued with full registration for a fee of UGX 100,000 and is thereafter renewed annually online. Renewal requires at least 48 CPD points on the practitioner's online account. Payments made after 31 December attract a penalty of 50% of the fees. The renewal fee is UGX 100,000 for a general practitioner and UGX 200,000 for a specialist.

A doctor who misses the deadline has a lapsed APL and a 50% surcharge, and the clinic has a renewal file with an invalid licence copy in it. Both are avoidable with one reminder in November.

Other registrars

Facilities supervised by nurses or midwives, or by allied health professionals, fall under their own councils rather than the UMDPC, and the requirements differ. The UMDPC document also notes that online courses and observerships in clinical disciplines are not registrable for CPD, while online health-related courses in non-clinical disciplines are registrable under conditions, which matters when a doctor is counting towards 48 points in December.

A clinic owner's annual file

Keep one folder, physical or digital, per licensing year containing: the business registration certificate, the inspection checklist, the supervising doctor's commitment letter and APL copy, the previous operating licence, the fee receipts, and the CPD evidence for each clinician. When the renewal window opens the file is already complete, and the only task is the inspection.
